Problem Solving Sheet

Students fill out this sheet after experiencing a behavior problem in class. Use this activity to help your students be able to express emotions, concerns, and the events of the problem clearly. By using a document, not only do you have a paper trail for keeping track of student behavior, but students also will feel like they have some say and control in their behavior and the outcomes for it.
