Substitute Teacher Feedback Form

One of the challenges we face when we have a substitute teacher is not having a clear picture of what took place in our classroom during our absence. It can be extremely helpful to get information about your students' behavior, and what was accomplished in the classroom. When you have this information you can plan accordingly for the next day, and hold your students accountable for any issues that occurred. It also helps the substitute teacher feel appreciated and valued. A teaching tip is to print out several of these forms so that you can easily pull one out and have it ready in case you get sick and need coverage.
