This resource is a worksheet for students to fill out so that you as the teacher can get some insight into how they feel about writing. This worksheet lets kids feel safe to share their true feelings about writing and allows them to reference their strengths and weaknesses in this subject. As a teacher, it can be very helpful to have an idea of who loves or hates writing, and to learn some more about the kids early on in the year. I would use this resource with grades 3 and above. My recommendation is to allow for student flexibility and let them know that this worksheet is to help you as a teacher better teach the kids, so honesty is always appreciated, even if the kids are saying they can't stand writing.
